Uncertified copies of birth or death certificates are no longer available.
Mother's Certificates are no longer issued
The Board of Health adopted the new fee for a
certified birth or death certificate to be $20.00 effective September
12, 2005. This change reflects the new state statute under Ohio Revised Code 3705.242.

All of the Marion county birth and death certificates are filed in the Vital Statistics office at the City Health
Department which is located on the 3rd floor of City Hall, 233
West Center Street.
Birth and Death
Certificates are filed in the county where the actual birth or
death occurred - not where the person resided at the time of
the birth or death. You may also obtain a birth or death
certificate by mail. Be sure to include the Application
and payment. For quickest service, include payment in
the form of a money order and enclose a self-addressed stamped
business size envelope

Download and Print the form below. Fill out form
completely, and bring or mail to the City Health Department: 233
West Center Street Marion, Ohio 43302. Each copy
is $20.00. Birth and death certificates are also now available
